How To Join The Scheme

There are two ways to be referred onto the Heart Smart Scheme.

Hospital Referral

Individuals must have completed within the last 3 months the Phase 3 Cardiac Rehabilitation programme at one of the 3 University Hospitals in Leicester.

If you are eligible the Cardiac Rehabilitation staff will complete a Phase IV referral form for you, which will be sent to the Cardiac Rehabilitation Officer at Charnwood Borough Council. *

Referral From Your g.p.

If you have been diagnosed with Coronary Heart Disease (CHD), and meet the eligibility criteria contact your G.P/ Practice Nurse. If they feel it is appropriate they will complete a Heart Smart Exercise Referral Form, which will be sent to the Cardiac Rehabilitation Officer at Charnwood Borough Council.

On receipt of your Phase IV referral form or your Heart Smart Exercise Referral Form the Cardiac Rehabilitation Officer will contact you to arrange an induction.

Previously participants on the Heart Smart Scheme could only be referred by the University Hospitals of Leicester. However, Charnwood Borough Council was selected by the British Association of Cardiac Rehabilitation (BACR) to take part in a National Pilot Project, allowing G.P's to refer eligible patients onto Heart Smart.

At present only selected G.P. Practices from North/South Charnwood are involved with the Heart Smart Scheme.

• The Surgery - Loughborough
• Birstall Medical Centre - Birstall
• Quorn Medical Centre - Quorn
• Alpine House Surgery - Mountsorrel
• Greengate Medical Centre - Birstall
• The Banks Surgery - Sileby
• The County Practice - Syston

Participants are referred for a period of 6 months. When their referral period has finished participants are encouraged to join the Isospa Fitness Suite at Loughborough Leisure Centre, Soar Valley Leisure Centre at Mountsorrel or South Charnwood Leisure Centre at Syston.
