b. The total number of businesses that you have conducted slip resistance testing upon
We do not store information specifically on slip resistance investigations and would need to go through individual accident notification records to determine if testing was carried out. However officers are not aware of any testing carried out in the last 24 months.

c. The methodology of slip resistance test carried out   0

d. The resultant figures obtained from tests carried out 0

e. The follow up action carried out as a result of tests n/a

f. The total number of reported slip accidents that you have received.
There were 20 slips trips and falls notifications in 2011/12 of which 4 were slips, none required any formal action or slip testing.
In 2010/11 there were also 20 slips trips and falls notifications of which 6 were slips, similarly none required any formal action or slip testing.

All notifications are reviewed by an Environmental Health Officer against a RIDDOR Decision Criteria Recording Form. Where necessary an investigation will be carried out and appropriate action taken.

We have in the past (2008 to 2010) targeted slips and trips in food premises during scheduled food hygiene inspections and supplied proprietors with an associated fact sheet on the topic.
